According to NY State Law, in order to register your child/children in the School District, you must be physically domiciled at your address within the Riverhead School District’s geographic boundaries. All proofs of residency must be current, within 30 days of registration.

The following documents are required to register your child/children:
- Student Documentation (1 of the following) Birth Certificate, Record of Baptism or Passport (other documents may be considered if none of these are available).
- Current Physical & Immunization Record
- Consent for release of school records and students most recent transcript, report card and IEP if applicable.
- Proofs of Residency (Landlord Affidavit, if applicable). See chart on page 2 Section A and B. You must provide at least one item from Section A and two items from Section B. If you cannot provide 1 item from Section A, you will need to provide three items from Section B. Click here Required Proof of Residency Document Chart
- Photo Identification
- Proof of Guardianship (if applicable)

- Signed Consent for Release of School Records
- Residency Questionnaire
