Instructional Support Team Forms

RHS IST Referral Form (doc)
RHS IST Referral Form (pdf)

Riverhead High School Instructional Support Team (IST)
Purpose:
The responsibility of the Instructional Support Team (IST) is to support teachers by identifying strategies and interventions to address the needs of students with academic and behavioral difficulties.  Through the process of Response to Intervention (RtI) problem solving, the IST collaboratively creates an intervention plan to address these areas of concern.  The Instructional Support Team will carefully monitor the effectiveness of the IST plan and recommend possible further interventions or revisions.

Members:

The Instructional Support Team (IST) is coordinated by building administration and includes student’s core academic teachers, guidance counselors, school psychologist, social workers and other support personnel as needed.

Procedures for Referral to IST

  1. Teacher should address concerns with parents of student.  A parent/teacher conference should be set up to brainstorm ideas to help improve performance.  The teacher should document all interventions attempted at the classroom level (TIER I) and dates of parent contact .  Student data collection should begin (i.e., work samples, formative/summative/NYS assessments scores).
  2. If student concerns persist, the teacher will complete the IST Referral Form and submit to the Guidance Counselor assigned to that student.  The teacher will work with the Guidance Counselor to collect necessary data and documents required.  Incomplete forms will be returned to each team before a student can be placed on the IST agenda.
  3. Twice per month, the Instructional Support Team will meet to brainstorm possible strategies and interventions to resolve areas of concern.  During IST meeting dates, teachers are expected to attend the IST meeting.  The IST will meet every 1st and 3rd Wednesday of every month, periods 7-9.
  4. The IST will document interventions and their effectiveness.  Specific data is needed to support evaluations (i.e., grades, examples of student work, incentive agreements, etc.)
  5. Student response to the intervention plan will be monitored and evaluated in 4-6 week cycles.  The IST will make further recommendations or revisions.
  6. Completed IST forms will be kept on file and reviewed by each Administrator assigned to that student.
